The USA Government, Mine Safety and Health Administration MSHA has reported a fatal incident that a haul truck that travelled backwards and overturned.
On 19 Jan 2021, a miner backed a haul truck to the edge of a dump point that was over steepened by a loader removing material at the bottom of the slope. When the edge of the bank failed, the haul truck travelled backwards and overturned, landing on the roof of the cab. The miner was fatally injured.
The Administration indicated the best practices:
- Always dump material in a safe location. If ground conditions aren’t reliable, dump loads a safe distance back and push the material over the edge.
- Never load material from the toe directly below an active dump point. This may lead to an over steepened and unstable slope.
- Never drive haul trucks beyond cracks on the top of the dumpsite.
- Always construct substantial berms as a visual indicator to prevent overtravel. Clearly mark dump locations with reflectors and/or markers.
- Always wear a seatbelt.
- Install advanced systems that restrain miners during roll-overs.
- Maintain communication between equipment operators and loaders.
- Train miners to use safe dumping procedures and recognize dumping hazards such as material slides and other unsafe conditions.

The USA based Mine Safety and Health Administration (MSHA) has reported a fatality that has occurred during the preparation of a mobile jaw crusher for shipping off site.
On August 26, 2020, two miners were preparing a mobile track mounted jaw crusher for shipping off-site. The crusher was missing the upper wrist pin from the hydraulic cylinder that raises and lowers the right hopper extension. The right hopper extension was secured in place by wedges. The victim was removing wedges, and when a wedge was removed, the extension fell, crushing the victim.
I am part of the project team in the process of terminating the Quebrada Blanca copper mine expansion here in northern Chile. The mine operator is Teck (Canada´s biggest miner) and this is a huge copper based project that will top US$6B.
We are terminating the works on the primary crusher FLSchmidt Gyratory and specifically the haul truck dump pocket. To this end, we want to get the haul truck wheel stop geometry correct.
From my days in mining projects in Australia in the 90´s, I recall two graphic photos, 1 of a large size FEL in the dump pocket of a mine – the name of eludes me. The 2nd was that of a haul truck in the dump pocket.
I´d dearly like to grab the photo of at least the haul truck with the aim of communicating to the design team the importance of the size and geometry of the wheel stop. Secondly, the photo would be used for Op´s training purposes.
I was wondering if any of your team has this in their archives. I have searched the web but to no avail.
